Nexus Counseling Center Systems TERMS AND CONDITIONS OF USE

By agreeing to the following terms and conditions, I acknowledge that I am requesting access to portions of my health information and the ability to communicate with my Nexus Counseling Center health care team concerning my health information via the Internet using an electronic application used by Nexus Counseling Center.

I understand that the Nexus Counseling Center system is intended as a secure online source of confidential medical information. I must have internet access and my computer's internet browser must be either

Google Chrome 52.x and above

I understand that Nexus Counseling Center System is never to be used as a means of communication to health care providers for urgent or emergency matters. For all emergency matters that I believe might immediately affect my health or well-being, I will dial 911 without delay and/or go to the emergency department of a local hospital.

I understand that my Nexus Counseling Center health care team may send me messages via the Nexus Counseling Center System. These messages may contain information regarding my health and medical care. It is my responsibility to monitor these messages that are sent to my Inbox. I agree not to hold Nexus Counseling Center liable for any loss, injury or claims of any kind resulting from messages that I fail to read or act upon in a timely manner.

I have the option of entering a valid and functional email address that may be used to notify me of the presence of messages in my consumer portal. If I opt to provide this email address for notification of the presence of messages, I will update my email address on email address changes.
<br /><br />
I agree that it is my responsibility to select a confidential password, to maintain my password in a secure manner, and to change my password if I believe it may have been compromised in any way. If I share my Nexus Counseling Center ID and password with another person, that person may be able to view my health information, and information about someone who has authorized me as an Nexus Counseling Center proxy. I agree not to hold Nexus Counseling Center or any of its subsidiaries liable for any loss, injury or claims of any kind resulting from my disclosure of the confidential password or confidential information on the Nexus Counseling Center system.

I understand that the Nexus Counseling Center system contains selected, limited medical information from a patient's medical record and that Nexus Counseling Center does not reflect the complete contents of the medical record.

I understand that my activities within Nexus Counseling Center may be tracked by computer audit and that entries I make may become part of the electronic record.
<br /><br />
I understand that access to the Nexus Counseling Center system is provided by Nexus Counseling Center as a convenience and that Nexus Counseling Center has the right to deactivate or restrict access to this system at any time for any reason or no reason without notice to me. I understand that my use of Nexus Counseling Center must be appropriate and may not be used to harass, intimidate, defame, or threaten any individual or for any illegal activity.

I understand that use of the Nexus Counseling Center system is voluntary and I am not required to use Nexus Counseling Center or to authorize a Nexus Counseling Center proxy.

I understand that the Nexus Counseling Center system may not be available to me all the time due to system failures, back-up procedures, maintenance, or other causes beyond the control of Nexus Counseling Center. Access is provided on an "as-is, as-available" basis and does not guarantee that I will be able to access the Nexus Counseling Center system at any particular time.

I UNDERSTAND THAT NEXUS COUNSELING CENTER TAKES NO RESPONSIBILITY FOR AND DISCLAIMS ANY AND ALL LIABILITY ARISING FROM ANY SOFTWARE INACCURACIES OR SOFTWARE DEFECTS, COMMUNICATION LINES, VIRTUAL PRIVATE NETWORK, THE INTERNET OR MY INTERNET SERVICE PROVIDER (ISP), ACCESS SYSTEM, COMPUTER HARDWARE OR SOFTWARE, OR ANY OTHER SERVICE OR DEVICE THAT I USE TO ACCESS the Nexus Counseling Center system.

During times when the Nexus Counseling Center system is unavailable, other communication methods (e.g., telephone) should be used to access Nexus Counseling Center.
